This modification includes the separation of the provider enrollment and customer service departments so we can concentrate our efforts on meeting your specific needs. A special team has been created to provide technical help desk assistance to you, our VIP clients. We are drawing on the strengths of our associates who were formerly with the second and third level support teams. For your convenience we have added web forms to the Provider Enrollment page on our website, address listed above. These forms are designed to help complete your service request in a more timely manner.
For Enrollment Requests please follow these convenient instructions:
The combined PEF (Provider Enrollment Form) can be downloaded from our website at http://www.medavanthealth.com/resource_center.aspx?id=99
All completed Provider Enrollment Forms (PEF) should be sent to the e-mail address listed above. We will respond within 2 hours if received by 3:00PM EST with a call tracking number. Payer agreements must be completed first. All payer agreements are available on our website at http://www.medavanthealth.com/resource_center.aspx?id=99
Please send the agreements to the recipient listed on the Instruction Page. Upon completion of the agreement, send the completed PEF to the above fax number or email address. MedAvant will reply with a notification of completion of data entry to your contact person within 24 to 48 hours of receipt of the PEF. Please remember that the form must be completed in entirety to avoid any processing delays.
If you do not find the Payer agreement you require on the MedAvant Website please contact the Provider Enrollment Team and we will forward the agreement to you within 48 hours.
The Provider Enrollment team will begin checking approvals within the time period set up by MedAvant for each payer once the PEF is received, which is generally 2-3 weeks from the date the payer agreement is mailed. Once the approval has been obtained from the payer and loaded into the MedAvant enrollment system, you or your client will be notified and may begin transmitting electronic claims to the specified payer on the date listed in the approval letter.
